Medical Director / PCP

Job Summary:

Provide direct primary medical care to all participants of the program following applicable policies, procedures, protocols, and Quality Improvement Performance Improvement (QIPI) Plan.

Duties/Responsibilities:

  • Performs interval history and physicals on established participants; completed at intake and quarterly or as indicated.
  • Evaluates and treats Participants with episodic illnesses.
  • Refers Participants to and oversees care by medical specialists as indicated.
  • Maintains required privileges with contracted and ad hoc facilities to maintain continuity of care.
  • Responsible for coordinating discharge planning with the team.
  • Manages all care of Participants in the nursing home.
  • Performs telephone visits with nursing home staff as required.
  • Admits nursing home Participants to the hospital when necessary.
  • Actively participates in family conferences.
  • Participates in team meetings, committees, and work groups as assigned.
  • Communicates participant changes to team members.
  • Participates in the development of the IDT Plan of Care.
  • Participates in the coordination of a 24-hour care delivery.
  • Completes all documentation in the medical record as required.
  • Work collaboratively with the Chief Executive Officer, Human Resources, and Executive Director on hiring and discharge of Physicians and Nurse Practitioners.
  • Work collaboratively, including prescriptive authority, with nurse practitioners.
  • Other duties as required.

Supervisory Responsibilities:

  • Oversee the QIPI program and staff.

Required Skills/Abilities:

  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must be able to deal tactfully with personnel, Participants, family members, visitors, government agencies/personnel, and the public.
  • Ability to organize work and follow through on assigned tasks.
  • Excellent written and verbal communication skills.
  • Must have the cognitive ability to process and perform tasks for Participant safety.
  • Attends and participates in continuing educational programs.

Education and Experience:

  • Graduate of an accredited School of Medicine with at least 2-3 years of clinical experience and one year of experience working with the frail and elderly population.
  • Able to supervise MD’s, DO’s, Nurse Practitioners, and other clinical professional staff.
  • Must maintain privileges at all One Senior Care and affiliates provider hospitals.
  • Must have proven clinical and administrative leadership abilities.
  • Ability to deliver service to physicians, patients, and fellow employees with compassion in a responsive, courteous, and concerned manner following PACE and Pre-PACE policies and procedures.

Physical Requirements:

  • Works inside the facility throughout the Participant and office areas.
  • Sits, stands, bends, lifts, and move intermittently during working hours.
  • Is subject to frequent interruptions.
  • Must be alert for Participant’s safety.
  • Is subject to lifting, carrying, and supporting Participants.
